After completing the Accreditation Requirements, the National Committee on Urban Pest Control [NCUPC] has submitted them to the Standard & Inspection Division of the Professional Regulation [PRC] for our CPD Accreditation Renewal as Continuing Professional Development [CPD] Provider following the CPD Matrix as provided for in the CPD Implementing Rules & Regulation [IRR] issued by the Commission. After the assessment, we are asked to pay the Accreditation Fee of P 5,000.00. Our CPD Accreditation Application is duly received.

Notarized Application Form with the following supporting documents in three [3] copies plus a receiving copy in folders and fasteners for the renewal of our Accreditation as CPD Provider are complete:
1). CPD Activities for the last three [3] years with CD and pictures;
2). Training equipment and facilities;
3). Annual Plan of Proposed CPD Activities;
4). General Information Sheet of the NCUPC for 2016;
5). Amended Articles of Incorporation & By-Laws;
6). Appointment Paper to manage the CPD Activities; and
7). Notarized Affidavit of Undertaking.
Processing of accreditation takes 30 days to complete. Anytime PRC may require us for additional papers.
